Case Management Flowsheets

Location: Client case management encounter screen

Function: bubble fields and reminders that track standards of care. E.g. for housing, reminding you that you need to do a housing assessment, housing application, and keep track of housing visits. These have been set up consistent with the City of Toronto Shelter Quality Assurance workflow. However it can also be configured for other quality of care workflows like diabetes management etc.

Reports

The following new reports are now available in CAISI.

Registration Intake Report

Location: Program Management Screen. (this feature has bugs and will be repaired and fully functioning in an upcoming software patch between Oct 29 -31)

Function: This tool will report on the information that has been collected in the agency's intake for a set period of time. The provider must input a start and end date for the report. Anything that is asked on the intake will be measured in this report, i.e. how many total intakes complete, how many client indicated addictions as a problem, how many clients have disability, etc.

Population Report

Location: Program Management Screen.

Function: This tool will report stats based on encounters made by various providers in various programs. The tool counts the number of encounters made by different roles for different issue areas like physical and mental health, addictions, client ID etc. It makes reports for individual programs. The report can be cut and pasted into a spreadsheet.

Client List Report

(this feature will appear in an upcoming software patch between Oct 29 -31)

Location: Program Management Screen, see link in the navigator bar

Function: you can identify clients seen by a provider in a particular program between particular dates.

New Features

  1. Program Signature. After you make a change to the program, system will save your name and the date.
  2. Batch discharge the clients from service programs.
  3. Only create a new client from PMM/New a client.
  4. doctor OHIP billing number shows up on the billing report.
  5. Disable billing link from case management view.
  6. Link to client's CME from the Queue of a program
  7. Large tabs on the top of Case Management Screen have been modified to smaller links
  8. Ticklers -(+) sign beside ticklers will now be a number representing how many ticklers the provider logged in has
  9. Link Service Programs to a Bed Program - this will create a relationship whereby if the two programs are linked, the client will be discharged from the Service Program when admitted to the Bed Program
  10. Log out Button now available on the case management screen and on the program management screen
  11. Custom Search of Notes: in CME click on search tab, a series of filters allows the provider to custom search for notes
  12. Bed Management: Reserve a bed for your client if they are in a bed program. Providers can set the expiry date on the reservation.

 

City of Toronto Requirement Features

  1. The ability to admit children along with their parent(s)- service provider can perform joint program admissions and discharges
  2. Access levels at more granular level - permissions to perform activities in system can be restricted/permitted at might higher granular level (i.e. staff can be restricted to only accessing intakes, not performing program admissions, discharges or accessing notes)
  3. Service Restriction: A program related service restriction functionality has been created in the system. Service restrictions are applied to a client and are  based in a program. The restriction records the staff who imposed it, any comments and the start and end date of the restriction
  4. System Overrides: Staff with the assigned permission, can override restrictions in the system -such as a service restriction
  5. Merging Duplicate Files: If there are duplicate files of the same client staff may merge the multiple files into one - if they have the access-level permissions to do so
  6. Client Search: The Search Engine in CAISI has been enhanced to include more variables, including program, admission to and admission from dates.
  7. Agency/Facility Configuration: the System configuration is now set up to have an agency and then facilities within that agency. For example, the City of Toronto could be an agency with the various shelters that fall under that umbrella being categorized as facilities.
  8. Gender Restrictions: Some facilities will admit 'men' or 'women' only, or 'women and transgendered' only. In customizing the system, service provider can create restrictions by gender for a specific facility or program in regards to Admissions or Referrals
  9. Age Group Restrictions: Some facilities will admit clients who are within a certain age range (for example, between 16 - 24). In customizing the system, service provider can create restrictions by age group for a specific facility or program in regards to Admissions or Referrals
