With user created (intake or other types) forms (you can add any questions you want to create a detailed intake for a shelter or homeless service) see the attached excerpt from a shelter intake form.
User created forms can be used in different parts of the OSCAR CAISI system (e.g. in the case management screen) or in the program management screen of a client record. Data collected in these forms can be used to generate reports. An unlimited number of questions can be generated and these forms can be version controlled.
