<body> <H2>Here are the steps to follow</H2> <p> 1. Save the files to your hard disk using the exact names provided, including upper and lower case as indicated. This is very important because the html files refer to these images and the tags embedded use these exact names.</p> <p> 2. Under Admin tab in OSCAR, click on “upload an image” and then click on browse. Find the image files you just saved and upload them all, one at a time. </p> <p> 3. Now go to “upload form” and upload each of the html files. Name each as you like (for PapReq for instance you may want to call it Pap Requisition) NB - this is _not_ the file name but just a name to give to the eForm, that you will see when choosing it from a list. </p> <p> 4. DONE! </p> <p> 5. Open a chart, check out eForms and your new forms should be there. </p> </body>
