This is a form for recording a complex care plan - billing codes 14030 14031 and 14033. It is a standalone form ie it doesn't require an image file to be uploaded with it. Contributed by Shelter Lee, Crossroads Family Practice, Chilliwack, BC.

Complex Care Plan (version 2)

This new version of the complex care interactive eForm incorporated research by Dr. Cam Ross/Quentin Smith/Bevan Voth into the form (just click on the link and the evidence pops up). Just click on one of the diagnosis at the top and the corresponding outline/targets/goals/handout lists would pop up. Select the handouts you want to print out at the bottom, and it'll spit out the printer when you click on the print button. This form is a monster, you'll have to upload the html file into the eForms, and all the files in handout.zip into the images. You'll also need your vendor to upgrade your eForm size limit to use this eForm. This form was developed for Firefox, and does NOT work well on Internet Explorer.
