CT scan - IV contrast related forms

Some forms used to satisfy the radiology department's concerns about the use of IV contrast in patients with renal dysfunction.

IV contrast Patient Handout: Contributed by Dr. David Page. Handout on the use of IV contrast and risk of renal toxicity. Also talks about the need for mucomyst and GFR testing.
Mucomyst Prescription: Based on the standard mucomyst prescription made by the Fraser Health Authority. You'll need the html file and the image file.
Updated Mucomyst instructions and Rx: This combines both patient instructions and prescription into a single e-form. Thanks to Peter Hutten-Czapski for figuring out how to combine multiple pages into a single e-form. You need the HTML file loaded into e-forms, and the image/png file loaded into image library.
