Lab Requisition with a "shortcut menu" that lets you click and automatically fills in a set of commonly ordered labs. You'll need to upload both the html file and the image file.
Lab Requisition
This lab req has a floating menu that allows you to choose, for example, 'Diabetes' which causes the lab requisition to be automatically filled with the appropriate tests! This is the html file - you need to upload this to OSCAR with the Lab Requisition Image file that goes with it. Contributed by Shelter Lee
Prenatal Lab requisition
Pre-filled BC lab req for early pregnancy. This eform uses the standard lab req image, which you likely already have if you have the lab req installed - if not then download the lab req image (LabReq.png) from the Lab Requisition folder (the same folder you are in now).
Lab Requisition - updated for Lower Mainland
This requisition is based on the original developed by Shelter Lee. Inspired by Herbert Chang to add drop-down boxes for locations, I have included most of the BC Bio and Life Lab locations. There is also a "fasting" instruction box, other categories, and a "copy to patient" checkbox. It is easy to customize - you can resort the lab locations to your local sites, so that they appear at the top of the list. It uses the same png image file as the previous form. And, finally, I have incorporated Shelter's larger checkbox feature!
