Lab requisitions must be in this new format. This form has built in references via highlighted items, customizable location selector, new menu items, time stamp for culture specimens, and future therapeutic drug monitoring items. It requires a new background image: LabReq2011.png.

- Upload this file to your image library. It must bear the exact name, which is case sensitive. Do not delete any of your existing lab images, as these are required to view any old lab forms already in your system.
- SOPLR with dark check marks
- Upload this file to your e-form library. It uses the same background as the other SOPLR. You will also need the file, jsgraphics.js, in your image library for the darker checkmarks to appear and print.

- This file is also used in the surface anatomy e-form, and others which have the darker checkmarks feature. You may already have this in your image library. It must be saved with this exact name. If Oscar does not allow you to save it, you already have this file.
