How to download eForms
Instructions on how to download eforms and then upload them to OSCAR.
Here are the steps to follow
- Find the form you are interested in in the list of e-forms (e.g. Joint Diagram)
- Save all the files in the folder to your hard disk using the exact names provided, including upper and lower case as indicated. This is very important because the html files refer to these images and the tags embedded use these exact names. (e.g. save both RheumatoidMan.png AND RheumatoidMan.html) Here are some tips.
- Under Admin tab in OSCAR, in the E-forms section, click on “upload an image” and then click on browse. Find the image files (e.g. RheumatoidMan.png not the html files) you just saved and upload them all, one at a time.
- Now go to “upload form” and upload the html file (e.g. RheumatoidMan.html). Name the form as you like (for RheumatoidMan.html for instance you may want to call it Joint Diagram) NB - this is _not_ the file name but just a name to give to the eForm, that you will see when choosing it from a list.
- DONE!
- Open a chart, check out eForms and your new forms should be there.
Note: you can do all of the above simultaneously for multiple forms to save time, ie not do it one by one for each form.
